The following commit changes behaviour so that hitting Cancel when asked
for the port number on which to add the server adaptor still results in the
server adaptor being added. That doesn't seem intuitive to me.

- register server adaptor with manager (that was not done previously in
some cases)
- use default port of the adaptor
