For information I got the same problem in NT 4.0 SP.5, Squeak 3.0 #4325.
To replicate:
-go to the 'Fun with Music' project,  play the ScorePlayer and after
that, try the following

MIDIFileReader playURLNamed:
		'http://squeak.cs.uiuc.edu/Squeak2.0/midi/wtellovr.mid'.

you'll end up with the same message: 
 'The specified device is already in use. Wait until it is free, and try
again. '

ant I second this 'I can asure you: Wait does not help.'
