I've also seen this happen before, but usually "do it again" works properly.  (It's working for me now as I test it.)

The trick here is to see if you can reliably reproduce the problem step-by-step, preferably in a fresh image.  Then, submit a [BUG] report, and there's a good chance someone will want to fix a bug like this.
