I missed the beginning of the thread, but here are a couple of more
things to try.

First, have you verified that sound is working in general on your machine?
eg, play a sound with sox and see that it works.  Maybe the mixer is set up
wrong, or maybe you don't have permissions on /dev/audio.

Second, you could try running squeak under "truss".  That way you
can get a log file of every system call Squeak makes, and then grep
through there for references to "/dev/audio".
