Did you try to get rid of the Worlds of Squeak by deleting the project view or executing the doIt located in (IIRC) the Welcome window.  If you are doing the former, then I have experienced this before.

I asked a question a while ago about this on the list but didn't chase it up.  There was a bug at that time with embedding projects within projects but not as direct morphic children.  In the Worlds of Squeak the ellipse morphs are an example of this.  The sub-projects are children of the ellipses and these are children of the World of Squeak project morph.  When you go to click on the X of the Worlds of Squeak view, there is some logic to check that none of its children are projects.  It doesn't let you delete the project if this is so.  However, this check isn't recursive and this is the problem.  The sub-projects get orphaned, disappear from view, but can still be accessed from the world menu.

Well maybe that's the problem :-)
