[SoC] Don't Forget Mid-term Evaluations

Don't forget students and mentors to fill out the mid-term evaluations.
The deadline is Monday at 19:00 UTC.  Each student have at least one
form to fill out, all mentors have a form and those with a student have
two.  Students and mentors with students must fill out their forms or
the student will, I believe, be dropped from the program without
payment.  So do it at your first opportunity.
