Thanks to the several people who pointed out to me that ctrl-esc is 
preempted by Windows.

Update 4594 now abandons ctrl-esc, and adopts instead Doug Way's 
suggestion of using *shift* esc as the universal gesture for directly 
bringing up the desktop menu from the keyboard, whatever the current 
context.
