Sure. The CocoaSqueak UI (outside Squeak itself) was done in InterfaceBuilder. Next steps to make this useful would be a mechanism for (a) passing messages from the outside to Squeak objects, probably via the event mechanism (lots of events left) and (b) publishing Squeak to the outside so IB can see them.
Extra credit for creating an IB palette, converting UI-descriptions back and forth, allowing Aqua UI elements to float on top of the Squeak view, getting FXBlt working, integrating native drawing (Quartz/OpenGL) etc.
