I only tried the Cocoa version briefly, and found it rather slow and 
a little incomplete (though this was at version 3.0.4; I think I've 
heard about some advances since then).

The Carbon version works great for me, though I've had it sometimes 
balk at a double-clicked image file I've never seen it refuse one 
entirely. What Carbon VM version are you using?
