Ned has it roughly right.  There is a page which describes how the 
harvesters work:

The harvesting process is going through a transition now, though, as 
the Guides take over certain responsibilities from Squeak Central, so 
that page is getting a little out of date.  As part of the transition, 
a new harvesting tool is being worked on, too.

Yes, although there will be more of a "fix" database first, to keep 
track of fixes/enhancements and make sure they get funneled into the 
next release.  Bug tracking capability could be added on later.  Some 
ideas on the future harvesting tool/database are here:
