[DOCS] SqueakMap entry examples (was RE: EventRecorder Steward?)

Those fields provided information related to the now defunct 3.3a
Modules system.  I don't think anybody needs to fill these out.. They
will probably go away in SM 1.1 -- right, Goran?

* Module path (ex: "People gh Killerapp"):

* Module version (ex: "1.2.4"):

* Module tag (ex: "CURRENT"): 

The other fields need no  additional explanation.
