Minor bugs in squeak 3.6 BFAV

The problem basically is that you cannot install via SqueakMap a package
that is not listed on SqueakMap.  And if you install something some way
other than via SqueakMap then it breaks the model.

Fixing this is on the todo list and will be done by not listing packages
in Package Loader that aren't flagged as 'installable' or something of
that sort.
