[squeak-dev] RFC: make ctrl-shift-d debug instead of duplicate (?)

Just for reference, Pharo uses Cmd-Shift-D for debug on Mac, and are
moving to Cmd=Ctrl on Windows.  I use it frequently and wouldn't want
to be without it (but never known any different)
