Ugh.  Did not mean to send yet.  Anyway...

That shows that a project's transcript is installed in Project newState:,
which assigns the project's transcript via TranscriptStream
class>>#newTranscript:. That implies that all normal transcripts are unique
to a project and have just one dependent (see TranscriptStream>>#open).

So the first thing to check is are all projects in your image holding onto
unique transcripts.  e.g. im my image

{Project allSubInstances size. (IdentitySet withAll: (Project
allSubInstances collect: #transcript)) size} #(5 5)
