One more data point:

I had similar problems as soon as I placed a new app behind an apache 
proxy.  Whenever I would upload large files apache would refuse to proxy 
the resulting page....no matter what was on it.  I found that turning 
off keepAlive in Kom fixed the problem (just look for implementors of 
keepAlive:)...or at least it seemed to.  Never had any problems with the 
app before I put it behind the proxy.
