Selecting mail out bug report from the debugger menu opens the DecentReviewerNotePad but another DecentReviewerNotePad window is attached at the bottom of the first one. Weird.
I have also noticed it is hard to bring other windows to the front of the desktop if there is a DecentReviewerNotePad active. Sometimes I have to move the window behind to bring it to front.
I'm using the BFAV2.11 version.

Yeah, I ran into that one too. It seems that 2.11 has some undesirable features that are being fixed.
Karl, the first bug (duplicate windows) has been reported and fixed in the latest version loaded out on squeak source which we are still testig. The second bug (other windows not coming forward) is new (to me) but definitely reproducible. Will work on trying to fix it. Tom

I submitted some changes that stuck it and the other BFAV GUI component into a real window, and these behaved much better.
So a morph isn't a window? I'll look at your changes.

No, but a window is a Morph.
In fact, there are a number of peculiarities about ordering and focus of SystemWindows that aren't consistent with the behavior of other Morphs.
