Smalltalk/V 286 was the successor to Smalltalk/V DOS. I worked on a distributed objects/multi-machine simulation system written in Smalltalk/V 286 back in 1989, and it had a full graphical environment.
You might be thinking about Methods, which was the predecessor to Smalltalk/V.
